High doses of acetylsalicylic acid may result in closure of foetal ductus arteriosus in utero and possibly persistent pulmonary hypertension in the new born. Kernicterus may be a consequence of jaundice in neonates. Do not take Alka-Seltzer XS if you are pregnant, breast-feeding or trying for a baby unless directed byyour doctor. Aspirin can prolong labour and affectblood clotting in the mother or baby.

The intake of acetylsalicylic acid by breast-feeding patients should be avoided as there is a risk of Reye's syndrome. Regular use of high doses could impair platelet function and produce hypoprothrombinaemia in the infant if neonatal vitamin K stores are low. Aspirin has also been associated with increased risk of Reye's Syndrome when given to children with a fever. For this reason the use of aspirin is not recommended in children under 16 years of age.

Do not take Alka-Seltzer XS for more than 3 days in a row. If symptoms persist, speak to your doctor orpharmacist. Salicylate poisoning is usually associated with plasma concentrations >350 mg/L (2.5 mmol/L). Most adult deaths occur in patients whose concentrations exceed 700 mg/L (5.1 mmol/L). Single doses less than 100 mg/kg are unlikely to cause serious poisoning. Forced diuresis should not be used since it does not enhance salicylate excretion and may cause pulmonary oedema.
