About this deal

A very small number of people using the vaginal ring may develop a blood clot in a vein or an artery. Don't use the ring if you've had a blood clot before. Cancer

Oestrogens may cause fluid retention and therefore patients with cardiac or renal dysfunction should be carefully observed. If you develop any of the conditions listed under ‘Do not use Estring’ if you have the following conditions’ or those listed under ‘Stop taking Estring and see a doctor immediately’ or if you have any of the following side-effects, remove Estring vaginal delivery system and see your doctor straight away. If you experience difficulty or pain when trying to remove the vaginal ring please do not continue and see your doctor. You can start using the vaginal ring 42 days after giving birth if you are breastfeeding. You will need to use additional contraception for 7 days (such as condoms). The Estring vaginal ring should remain in place for 90 days. It should then be removed and replaced by a new ring, if prescribed by your doctor. If at any time the ring falls out, rinse it with warm water and reinsert it. If it slides down into the lower part of the vagina, use a finger to reinsert it. You may be able to start using the ring earlier than this – on day 21 after giving birth – if you're not breastfeeding, but your doctor will advise you. You do not then need to use additional contraception. Generally recognised risk factors for VTE include, use of oestrogens, older age, major surgery, prolonged immobilisation, obesity (BMI > 30 kg/m 2), pregnancy/postpartum period, systemic lupus erythematosus (SLE), and cancer. There is no consensus about the possible role of varicose veins in VTE.

Adapted from numerous WHI publications. WHI publications can be viewed at https://www.nhlbi.nih.gov/whi/. † Results are based on centrally adjudicated data. ‡ Nominal confidence intervals unadjusted for multiple looks and multiple comparisons. § Not included in "global index". ¶ Includes metastatic and non-metastatic breast cancer with the exception of in situ breast cancer. # All deaths, except from breast or colorectal cancer, definite or probable CHD, PE or cerebrovascular disease. Þ A subset of the events was combined in a "global index", defined as the earliest occurrence of CHD events, invasive breast cancer, stroke, pulmonary embolism, endometrial cancer, colorectal cancer, hip fracture, or death due to other causes. For women in their 50s who are taking HRT, the figure would be 11 in 1000 , over a 5-year period (i.e. an extra 3 cases).Adults—At first, 4 micrograms (mcg) or one insert into the vagina once a day for 2 weeks, followed by one insert 2 times a week (every 3 to 4 days). Your doctor may adjust your dose as needed. Endometrial safety of long-term (more than one year) or repeated use of local vaginally administered oestrogen is uncertain. Therefore, if repeated, treatment should be reviewed at least annually, with special consideration given to any symptoms of endometrial hyperplasia or carcinoma. You can start using the ring immediately after a miscarriage or abortion, and it'll work straight away. You don't need to use additional contraception.

Do not use Estring if you are pregnant. Tell your doctor right away if you become pregnant during treatment. Endometrial overstimulation, as evaluated in non-hysterectomized patients participating in the U.S. study by the progestogen challenge test and pelvic sonogram, was reported for none of the 58 (0 percent) patients receiving ESTRING and 4 of the 35 patients (11 percent) receiving conjugated estrogens vaginal cream. You will leave the ring inside for 90 days (3 months), unless your doctor tells you a different schedule. After that time, remove the ring and insert a new one. If you forget to remove the ring after 90 days, call your doctor for instructions. You can also choose to have a shorter ring-free break or not to have a break at all. This is as safe and effective as the standard use. For more information on the different options talk to a GP or nurse.

F. Pregnancy

The Estring is a slightly opaque ring made of a silicone elastomer, with a whitish core, containing a drug reservoir of estradiol hemihydrate. You may need emergency contraception if you had sex before you remembered to put the new ring in, and the ring-free interval was 48 hours longer than it should have been or more (9 days or more in total). If the ring comes out by itself Signs of high blood pressure like very bad headache or dizziness, passing out, or change in eyesight.
