Magento Google XML Sitemap

Are you guys using Magentos built in sitemap system or some other system?

Are you bothered that it doesn't include all pages in the sitemap (eg reviews etc) or do you think that's a good thing not to include?

I prefer for magento stores to use the built in sitemap generator and not to include reviews. However should the client wish for them to be included i use a third party sitemap generator on a cron job.

The problem with standard Magento sitemap was solved. Now the sitemap includes all the site page (including the static ones!), they are displayed at one page, you can even filter the pages from the sitemap, Google sitemap now includes categories in the product URLs (if enabled). And Magento sitemap became well seo-optimized and user-friendly. This was done in the "Extended Sitemap" extension.
You can find it in Magento Connect

Generally, I'll include the important pages that I want to rank, but not reviews/accountlogin, or even every product page.

Make sure you're using the <priority> system effectively as well.

I use the builtin sitemap feature but I am somewhat concerned about the way <lastmod> is being included. The problem is, that magento uses the date the sitemap was created/modified last and fills all <lastmod> entries with that information. I think <lastmod> should only be updated, if a page or category actually changes...

Yes it really matters what pages to include in sitemap and what to not. Plus there are Magento extensions for generating sitemap.
