The poorer agreement in identification of ADHD ‘caseness’ for children with LL may appear at odds with previous findings that agreement between parents and teachers is often higher in clinical versus non-clinical samples [ 20, 50]. However, clinical samples are subject to Berkson’s bias and the children within our LL group were identified as having low language from a community sample. Thus parents and teachers in the current study have likely spent less time discussing the child’s progress, strengths and weaknesses, which in clinical samples may result in a consensus view of the child’s behavioural profile. An important caveat is the low return rate of questionnaires from both parents and teachers (overall 57% of parents and 65% of teachers returned completed SWAN questionnaires). Children without SWAN questionnaires from both parents and teachers had significantly greater language, behaviour and early academic weaknesses relative to those in the current sample (Additional file 1: Table S1). It is therefore possible that agreement would be enhanced for these children with more severe language and behavioural deficits. Furthermore, given there were low numbers of ADHD ‘cases’, particularly in the TL group, it is possible that with a bigger sample of children which includes more ‘cases’ of ADHD, parent-teacher case agreement would be poorer.
