Colecalciferol capsules should be prescribed with caution to patients suffering from sarcoidosis because of the risk of increased metabolism of vitamin D to its active form. These patients should be monitored with regard to the calcium content in serum and urine. Anyone who doesn’t get much exposure to the sun – such as people who work indoors, or are housebound, or who tend to cover up when they go outdoors – will be short of vitamin D, and will need more than the Department of Health’s recommended minimum of 10μg per day.

It is desirable for improving acceptability and compliance to offer different types of preparations of vitamin D 3. Today, there is no consensus whether any of the different vitamin D 3 preparations for oral use would be better than the other in raising serum concentrations of 25-OHD. Indeed, comparisons between the effects of supplementation with vitamin D 3 tablets versus oral oil drops in healthy adult patients are scarce, and in the few studies performed no administration method has been shown to be superior [ 14, 15, 16, 17].Colecalciferol and its metabolites are excreted in breast milk. Overdose in infants induced by nursing mothers has not been observed but allowance for any maternal dose should be made when prescribing vitamin D products to a breast-fed child. Studies have shown safe use of doses up to 4000IU during pregnancy although studies in animals have shown reproductive toxicity (see section 5.3). Adequate vitamin D intake is essential for maternal and fetal health during pregnancy, and epidemiological data indicate that many pregnant women have sub-optimal vitamin D levels. Notably, vitamin D deficiency correlates with preeclampsia, gestational diabetes mellitus, and bacterial vaginosis, and an increased risk for C-section delivery.
