8. Cautions with other medicines

The recommended dose depends on the type of diarrhoea you have and your age. Adults (over 18), with short-term diarrhoea or IBS If patients are taking this medicine to control episodes of diarrhoea associated with Irritable Bowel Syndrome previously diagnosed by their doctor, and clinical improvement is not observed within 48 hours, the administration of loperamide HCl should be discontinued and they should consult with their doctor. Patients should also return to their doctor if the pattern of their symptoms changes or if the repeated episodes of diarrhoea continue for more than two weeks. Non-clinical data have shown that loperamide is a P-glycoprotein substrate. Concomitant administration of loperamide (16 mg single dose) with quinidine, or ritonavir, which are both P-glycoprotein inhibitors, resulted in a 2 to 3-fold increase in loperamide plasma levels. The clinical relevance of this pharmacokinetic interaction with P-glycoprotein inhibitors, when loperamide is given at recommended dosages is unknown. Age 12 to 17 years with IBS or long-lasting diarrhoea – follow the instructions from their doctor. This is given on prescription only. If you miss a dose of loperamide, do not worry. Just take a dose after you next go to the toilet and have a runny poo.

Acute and chronic studies on loperamide showed no specific toxicity. Results of in vivo and in vitro studies carried out indicated that loperamide is not genotoxic. In reproduction studies, very high doses (40 mg/kg/day – 20 times the maximum human use level (MHUL)), based on body surface area dose comparison (mg/m 2), loperamide impaired fertility and fetal survival in association with maternal toxicity in rats. Lower doses (≥ 10mg/kg/day – 5 times MHUL) revealed no effects on maternal or fetal health and did not affect peri- and post-natal development. Other ingredients are: Gelatin, mannitol, aspartame, mint flavour and sodium hydrogen carbonate. What the medicine looks like If a doctor prescribes loperamide for your child, they will use their weight or age to work out the right dose. The dose also depends on their symptoms. What if I forget to take it? IMODIUM® Instant Melts are used to treat two types of diarrhoea. The two types have different age limits.

If you are pregnant, think you are pregnant or planning a pregnancy: ask your doctor or pharmacist for advice before taking this medicine. The concomitant administration of loperamide (16 mg single dose) and ketoconazole, an inhibitor of CYP3A4 and P-glycoprotein, resulted in a 5-fold increase in loperamide plasma concentrations. This increase was not associated with increased pharmacodynamic effects as measured by pupillometry. Two tablets (4 mg) initially, followed by 1 tablet (2 mg) after every loose stool, or as previously advised by your doctor. The maximum daily dose should not exceed 6 tablets (12 mg).Your body can lose large amounts of fluids and salts when you have diarrhoea. You need to replace the fluid by drinking more liquid than usual. Ask your pharmacist about rehydration therapy to replace lost salts. capsules or tablets: take 2 capsules or tablets, taken immediately. Then take 1 capsule or tablet after each runny poo.
